Upcoming Virtual Live Sessions For NH AHEC Health Equity Scholars

Lebanon Mobile Integrated Health Program

Monday, December 5, 2022
1:00–2:00 pm

The Lebanon Fire Department and DHMC Mobile Integrated Health program optimizes healthcare service delivery for Dartmouth Health patients living in Lebanon and West Lebanon, through providing chronic and preventative care to patients in their homes.  With a goal of reducing unnecessary emergency medical transports and visits, the team of paramedics and nurses provides care when, and where, it is needed most.

Amanda St. Ivany, Lebanon Community Nurse, will be joining the NH AHEC Health Equity Scholars to share some of what she has learned through her work with this mobile care model. In addition, she will explore a de-identified case-based scenario with us and present an opportunity for Scholars to volunteer with the program.
