The Lebanon Fire Department and DHMC Mobile Integrated Health program optimizes healthcare service delivery for Dartmouth Health patients living in Lebanon and West Lebanon, through providing chronic and preventative care to patients in their homes. With a goal of reducing unnecessary emergency medical transports and visits, the team of paramedics and nurses provides care when and where it is needed most.
