After Completing the DDD

Once you are satisfied with your responses to the Directive, follow these steps to ensure your directive is available and up-to-date should you ever need it:

Make sure the Directive document is available:

1. Make copies of the Directive and share it with your friends and family.

2. If you haven't selected a Durable Power of Attorney for Healthcare, you should do that as soon as possible.

3. Make sure your Durable Power of Attorney for Healthcare has a copy of your Directive.

4. Share the Directive with your physician.

5. Ask to have the Directive included in your medical record.

Consider taking the following additional steps:

6. Review a copy of this Directive with your attorney and provide a copy of the directive for his or her files.

Be sure to update your Directive periodically:

8. Set a reminder to yourself to revisit the directive every 1-2 years.

Complete this Record of DDD and Video Recording and save it with your important papers
