How to use the Dartmouth Dementia Directive

Begin by downloading the appropriate documents:

1. The Dartmouth Dementia Directive

2. For the Person Completing the Dartmouth Dementia Directive: The Dementia Staging & Background Document

3. For Agents and Medical Providers: Determining the Stage of Dementia Using the DSRS

Next, fill out the Directive document:

4. Take your time and think carefully about your wishes for care should you develop dementia

5. Complete the Directive

6. Speak with your friends and family about your decisions

Make sure the Directive document is available:

7. Make copies of the Directive and share it with your friends and family.

8. Make sure your Durable Power of Attorney for Healthcare has a copy of your Directive.

9. Share the Directive with your healthcare provider.

10. Ask to have the Directive included in your medical record.

       11. Download and complete the "Record of DDD and Video Recording" form,  and keep this form with your important papers.

Be sure to update your Directive periodically:

12.   Set a reminder to yourself to revisit the directive every 1-2 years.

13.   If you change any of your wishes in the directive, make sure that your  DPOA, family members, and healthcare providers have copies of the updated form.  Also, update your "Record of DDD and Video Recording" form.

14.  If you have any questions or concerns about the Dartmouth Dementia Directive, be sure to discuss these with your primary care clinician.
