We engage with primary care clinics through quality improvement initiatives, pilot redesign projects, practice-based research, and targeted education. Our partnerships are with clinicians, patients and communities in designing, monitoring and evaluating primary care innovation using a team-based approach.

Education & Training

Geisel School of Medicine at Dartmouth

Decision makers must be cultivated and empowered to use training tools to reflect the diverse communities they are meant to serve. We will work with organizational leaders to uplift the voice of primary care as foundational to our health care system leveraging our relationships with Dartmouth Health and the Dartmouth Geisel School of Medicine to expand high-quality primary care.

Northern New England CO-OP Practice & Community-Based Research Network (NNE CO-OP PCBRN)

Home to the oldest, active primary care Practice & Community-Based Research Network (PCBRN) in the country the Dartmouth CO-OP is known for its implementation and dissemination of research and care delivery for underserved communities.

The Levy Incubator

The Susan & Richard Levy Health Care Delivery Incubator is a joint venture between The Dartmouth Institute for Health Policy & Clinical Practice (TDI) and Dartmouth Health (DH) to support the development of innovative health care delivery solutions. The Incubator’s primary goal is to provide rapidscalable, and transformational redesign of health care services.
