There is growing data to suggest that both patient outcomes and care team satisfaction and well-being can achieve remarkable improvements through the social physics of a learning collaborative that includes professionals engaging with patients, families, and care partners. Our biannual Collaborative Learning Network Conferences will be a place where patients, care partners, and teams can meet to share their progress, learn about new research, tools, and innovations, and learn with and from one another. Our goal is to break down silos and spread innovative practices by building a robust interdisciplinary community of practice that learns, measures, shares, improves, and “steals shamelessly” to spread best practices. In 2021, we will explore how to create an environment that fosters team well-being and joy in work.
