Title/abstract follows FLASH Radiation Therapy at Dartmouth
Ultra-high dose rate (UHDR) radiation delivery, termed FLASH radiotherapy, has the potential to reduce normal tissue damage, when compared to conventional radiation delivery rates, at the same total dose. Two pressing issues with FlASH RT are; the mechanisms for this unexpected result remain unexplained by convincing data, and most currently available in vivo research data is largely based on studies rodents which often lack the morphology, variability and late effect situations that must be addressed in human patients. Dartmouth initiated the first experimental MeV FLASH beam, up to 300 Gy/s, that can be switched on and off within a clinically commissioned linear accelerator and has developed unique fast dosimetry tools to QA the beam and monitor the delivery. This presentation will focus on development of Dartmouth FLASH radiation system and the comparative FLASH vs Conventional RT rodent and large animal studies performed at Dartmouth and elsewhere. Specifically, we have conducted rodent and pig skin studies at 20-40 Gy (single dose) and have treated two spontaneous canine tumors at 5 x12 Gy. Assessment techniques include clinical observation, optical coherence tomography, immunohistochemistry and qualitative/quantitative histopathology. Post-RT pig skin and canine tumor studies, using single and fractionated doses, are ongoing, with some animals currently more than 6 months post treatment.
